A vehicle inspection checklist is a five-station list that a technician works in one pass: road test, under hood, under vehicle, lighting and glass, then documentation. The form is the document that holds it, and the difference decides whether a finding survives an argument. Below is the whole checklist. What does the form around the checklist need?
The item list above is what a technician works. The form is what survives afterward, and it needs four blocks. Drop the last one and you have a worksheet.

How should each item be rated?
Three ratings, and the shop decides the thresholds once so every technician uses the same ones. Green means the item was checked and sits inside the shop's own limit. Yellow means it was measured and the number is heading somewhere. Red means below the limit or unsafe now.
The rule that makes ratings useful is simple. A yellow or red line needs a measured value and a photo before it leaves the bay. An advisor selling a $900 brake job on the phone can read "three millimetres on the front pads, and here is the photo" to a customer standing in a parking lot four miles away. An advisor holding a checklist with a yellow dot next to "brakes" has nothing to say.
What does the regulation say the report must record?
For a periodic inspection on a commercial vehicle the answer is written down. Quoting 49 CFR 396.21 in full, the qualified inspector must prepare a report that:
"(1) Identifies the individual performing the inspection; (2) Identifies the motor carrier operating the vehicle or intermodal equipment provider intending to interchange the vehicle to a motor carrier; (3) Identifies the date of the inspection; (4) Identifies the vehicle inspected; (5) Identifies the vehicle components inspected and describes the results of the inspection, including the identification of those components not meeting the minimum standards set forth in appendix A to this part; and (6) Certifies the accuracy and completeness of the inspection as complying with all the requirements of this section."
Six requirements, and five of them are header fields. The fifth is the item block. Nothing in that list mentions a logo, a page border or the twelve-column grid the free Word templates lead with.
The daily report has its own shape. 49 CFR 396.11 is direct about what it has to say and who signs:
"The report must identify the vehicle and list any defect or deficiency discovered by or reported to the driver which would affect the safety of operation of the vehicle or result in its mechanical breakdown."
"The driver must sign the report."
The electronic question is answered in the same section:
"The report required by this paragraph (a) may be created and maintained in electronic format, in accordance with 49 CFR 390.32."
Retention splits by document. A driver vehicle inspection report and its certification of repairs are kept three months under 396.11. A periodic inspection report is kept fourteen months under 396.21, held where the vehicle is housed or maintained. Shop inspections outside those rules should live as long as the repair orders they sold, because the inspection is the evidence behind the sale.
What measurements does the federal standard require?
For a truck running under a DOT number, the numbers are published and there is nothing to argue about. Appendix A to 49 CFR Part 396 opens with the standard it is applying, quoted here from the regulation itself:
"A vehicle does not pass an inspection if it has one of the following defects or deficiencies:"
On brake lining, for non-steering axles:
"Lining with a thickness less than 1/4 inch at the shoe center for air drum brakes, 1/16 inch or less at the shoe center for hydraulic and electric drum brakes, and less than 1/8 inch for air disc brakes."
On tires, for any tire on a steering axle:
"With less than 4/32 inch tread when measured at any point on a major tread groove."
Every other tire on the vehicle fails under 2/32 inch by the same appendix. Read the brake sentence again and notice what is missing from it: air drum, hydraulic drum and air disc each get a figure, and a hydraulic disc pad does not. That is what most light trucks and every passenger car run on, so the threshold for those rows is the shop's own, and printing it next to the measurement is what stops two technicians rating the same pad differently.
The daily report on a commercial vehicle has its own item list. 49 CFR 396.11 requires a written report at the completion of each day's work covering service brakes, parking brake, steering mechanism, lighting devices and reflectors, tires, horn, windshield wipers, rear vision mirrors, coupling devices, wheels and rims, plus emergency equipment. The corrective-action paragraph is the one shops should read twice:
"Every motor carrier or its agent shall certify on the driver vehicle inspection report which lists any defect or deficiency that the defect or deficiency has been repaired or that repair is unnecessary before the vehicle is operated again."
What does a filled-in checklist look like?
A 2019 Ford F-250 with the 6.7L Power Stroke, 96,400 miles, in for a noise complaint and a maintenance inspection. This is an illustration and no line on it came off a real ticket. Thresholds in the right-hand column are the shop's own, printed on the form. Only the lines that produced something are shown.

Six lines are not green, and they group into four jobs. Both front pads and the scored rotors are one brake job. The front cover is a cooling repair. Then the belt, then the battery at 68 percent of rated. Ratings already put those four in the order they should be sold in, which is the second thing a threshold column buys you.
Why does a line pass the federal floor and still sell?
Because the two numbers are not close to each other. Take the same form filled in for an annual inspection on a 2016 Ram 3500 with the 6.7L Cummins at 141,880 miles, where Appendix A does bind:

One thirty-second of an inch stands between those steer tires and the point at which the truck does not pass an inspection. They clear the federal floor, and they are still tires nobody should send back out loaded. That gap is the most useful thing a shop can print on its own form. The federal number is where a vehicle goes out of service. The shop number is where the vehicle should get fixed. Put both in the threshold column and the conversation stops being about opinion.
What does one finding cost by the time it is priced?
Take the front brake line off the F-250 and price it the way the ticket will. Inputs are stated so every figure can be checked: the shop's canned job for front pads and rotors on this truck is 2.1 hours, the posted labor rate is $165.00 per hour, and the costs are illustrative supplier costs run through the bands in the parts matrix template.

Parts add to $529.82. Labor adds $346.50. The whole job hangs off one number a technician wrote down with a pad gauge, and that is the argument for making the checklist record measurements instead of dots.
What does the shop copy need that the customer copy does not?
Four things, and keeping them off the customer copy is what makes the customer copy readable.

The prior-values field earns its space fastest. Today's 4 mm reading on a customer copy is a fact and nothing more. A shop copy carrying the last two readings with the odometer beside each one gives an advisor a wear rate: millimetres lost, divided by miles between the readings. Multiply what is left by that rate and the recommendation stops being "sometime" and starts being a mileage, which the customer can put against their own driving.
On a vehicle running under a DOT number, run that subtraction down to the federal floor. The truck goes out of service there, well before the pad reaches bare backing plate, so zero is the wrong endpoint and it flatters the answer by thousands of miles.
Where do inspection checklists and forms go wrong?

Two of those are quiet in different ways. A shop that records only its yellows and reds throws away the baseline that makes the next visit persuasive, so the customer hears a new number every time with nothing behind it. And the deferred flag pays for itself the first time it fires: a customer who declined a $1,400 turbo job in March remembers declining it, and an advisor who re-pitches it in July with no acknowledgement that it is the same job loses credibility on everything else on the ticket.
How do you copy this into a spreadsheet?
Two sheets. The header block goes in a single row on the first. The second carries one row per inspection line, and the station column is what lets a tech sort it into the order they walk the vehicle.

What happens to the checklist after the tech marks it complete?
It becomes an estimate, and that is where a shop's day disappears. Somebody reads the findings, decides which ones are one job and which are two, looks up labor hours for the exact year, make and engine, chases suppliers for parts prices, then keys every line onto the ticket. Four findings on a diesel pickup means four passes through all of that while the customer waits on a callback.

Every other tire fails under 2/32 inch. Shops working non-commercial vehicles usually set a yellow threshold above the legal minimum so the customer gets warned before the tire is illegal.
- Does a green rating need a measurement?
- It helps more than people expect. A pad recorded at 9 mm today is what makes the 6 mm reading in nine months persuasive, because the customer can see the wear rate on their own vehicle. Green with no number gives the next advisor nothing to compare against.
- Who signs a vehicle inspection?
- The technician who performed it. On commercial vehicles the driver signs the daily report under 49 CFR 396.11, and the carrier certifies separately that any listed defect has been repaired or that repair is unnecessary. - Should the VIN be typed onto the form?
- It should be decoded, not typed. Seventeen characters keyed by hand at a busy counter produce a vehicle that does not exist, and every labor lookup downstream then returns a time for the wrong truck. Read it off the plate with a scanner, or decode it through the free NHTSA vPIC service, which returns the year, make, model and engine from the number itself.
- How does a checklist turn into an estimate?
- Each red or yellow line becomes a job, the job gets labor hours from your canned job or a published labor guide, and the parts get priced from your suppliers. That step is where the counter's day goes. It is also the step automated estimating removes, by reading the completed inspection and writing the priced draft onto the same repair order for a service advisor to review.
